Registration—Youth Clinic

WHO:  youth age 11 to 14 years

EQUIPMENT FITTING:  Saturday, July 12th; 8 am-noon

WHEN:  July 15-17 & July 21-24 (seven-day camp)

HOURS:  5:00 p.m. to 8:00 p.m. (please arrive 15 min. early)

WHERE:  Portsmouth Christian Academy

20 Seaborne Drive, Dover, NH

COST:  no cost

(150 available spots will be filled on a 1st-come, 1st served basis.  Your slot is not secure until all required paperwork is submitted)

While forms can be faxed and sent via

e-mail, this process will only “HOLD” your registration for five (5) days until your original paperwork, along with the player’s physician approval, is received.  Once the originals are received, a confirmation  letter will be sent.  Your “hold” will be released and your slot lost if paper-work is not received within the five days.  Medical insurance is required for each player.
